To be eligible for home care services you must be:
Senior age 65 and over or disabled person age 18 and over

You have limited abilities to walk or get out of your home

You require the assistance of others to leave your home when you do leave your home, it is to get medical care, or for short, infrequent, non-medical reasons such as getting your hair cut

You are confined to the home and unable to function independently

You have trouble bathing, dressing, etc.

You have chronic health problems

You have multiple medical needs

How to receive our services
Call 616 243-1400 to go through initial telephone screening to be assessed by a registered nurse and our representative to determine eligibility and answer all your questions including payment options for our services.

Submit all required income eligibility information to get approved for our services.

A registered nurse and a team of social workers will develop a care plan customized to meet our prospective client needs.

If your care plan is approved by our prospective client and/or caregiver, we will proceed with purchase of services

Reevaluation of the care plan will be conducted every two months to determine if you require additional services and/or receive care that you deserve.

Our services are free for those who are eligible

Emergency & same day care
Our staff is available 24 hours per day, 7 days per week, 365 days per year, including all holidays. Your needs may arise where immediate homecare may be necessary. We are available to meet your and care about you the moment you call us.

We serve all counties of Grand Rapids area without any limits. Our services will be available soon in greater Detroit area.
